Frankly, I dont think the world in general has ever made an effort to use the okina in either written or spoken language and I suspect the reasons are twofold. Also, people who have never been to the islands likely have no idea what to do with it; trying to pronounce the letters in humuhumunukunukuapuaa is enough of a challenge.

Second, typography doesnt always accommodate it easily so its often omitted, especially in street signs; then people learn words the wrong way and the mispronunciation becomes widespread. If you listen to Hawaiian radio or television you will be able to hear the okina. Learning to use it, for me at least, is a whole other thing.

As of 2016, there has been action to correct the official state name to include the okina but the proposal seems to be proceeding on Hawaiian time.
