Because in the States they do not sell a traditional european duvet. In Europe it's a cover that replaces a top sheet. Here it is a washable removable "pillow case" that houses a down comforter. Much more sanitary and usable. Who cares about the semantics. It's about sanitation and flexibility. I want to wash my top sheet weekly and most european duvets are difficult to put into a standard washing machine because of their size.
